Request for Disability Accommodation

Consistent with our mission and core themes, DCC provides academic accommodations to ensure a quality education to all students. If you wish to receive academic accommodations, please follow the steps below.

Process

  1. Complete the Request for Accommodations, Release of Information, and Disability Verification forms found below
  2. Provide all applicable documentation. Documentation may include:
    1. Individual Educational Plan
    2. 504 Plan
    3. Letter from qualified health care provider
    4. Any other official documentation that relates to your accommodation request
  3. The SDAS Administrator will review your request and contact you to schedule an interview to evaluate your academic accommodation request 

Initial Forms


Services include, but are not limited to:

  • Testing accommodations
  • Recording lectures
  • Note-takers
  • Instructor notes
  • Private testing rooms for eligible students
  • Double time on assignments, homework, projects and exams
  • Books in an alternative format

Additional Forms


Students with disabilities who request accommodations must:

  • Register with Student Disability & Accommodation Services 
    • It is the student's responsibility to initiate the request for services
  • Students who are requesting accommodations must provide official documentation of their disability from their medical or psychological professional

Documentation must:

  • Be current
    • Current is variable depending on the nature of the disability. Timeframes will be considered on a case-by-case basis.
  • Include a specific diagnosis
  • Include actual test scores if applicable
